While many people remain asymptomatic, complications can arise, including diverticulitis (inflammation or infection of the diverticula), abscess formation, perforation of the colon, peritonitis, fistulas, and intestinal obstruction. Diagnosis typically involves a combination of clinical evaluation, imaging studies, and sometimes colonoscopy. Colonoscopy or Virtual colonoscopy (a specialised CT scan of the abdomen and pelvis) is the most sensitive and specific tool for identifying diverticulosis.
Differentials to consider include appendicitis, inflammatory bowel disease (Crohn’s disease or ulcerative colitis), colorectal cancer, ischemic colitis, and infectious colitis, as these conditions can present with similar abdominal pain, fever, and altered bowel habits.
